Amoxicillin trihydrate
Indications/Uses
Susceptible bacterial infections of ear, nose, throat (eg, pharyngitis/tonsillitis, otitis media, rhinosinusitis); lower resp tract including community-acquired pneumonia; GUT; skin & skin structure; other infections. In combination w/ other drugs for H. pylori eradication. Prophylaxis of bacterial endocarditis.
Dosage/Direction for Use
Adult & childn ≥40 kg Mild to moderate infection 500 mg every 12 hr or 250 mg every 8 hr. Severe infection May increase dose up to 875 mg every 12 hr or 500 mg every 8 hr. Lower resp tract infection 875 mg every 12 hr or 500 mg every 8 hr. Community-acquired pneumonia Combination w/ macrolide or doxycycline: 1 g tid. Max duration: 5 days. Simple acute UTI Two 3 g doses w/ 10-12 hr intervals. Gonorrhea 3 g as single dose. H. pylori GIT infection Combination w/ PPI & clarithromycin or metronidazole (triple therapy) or PPI, clarithromycin & metronidazole (quadruple therapy): 1 g bid for 14 days. Prophylaxis of bacterial endocarditis 2 g 30-60 min before procedure. Childn <40 kg Mild to moderate infection 125 mg every 8 hr. Severe infection May increase dose up to 250 mg every 8 hr. Acute otitis media 750 mg bid for 2 days may be used as alternative course of treatment. Prophylaxis of bacterial endocarditis 50 mg/kg 30-60 min before procedure. Renal impairment w/ GFR 10-30 mL/min 250-500 mg every 12 hr, <10 mL/min 250-500 mg every 24 hr. Hemodialysis 250-500 mg every 24 hr after dialysis.
Administration
May be taken with or without food.
Contraindications
Serious hypersensitivity (eg, anaphylaxis & SJS) to amoxicillin or other β-lactams (eg, penicillins, cephalosporins).
Special Precautions
Discontinue if severe anaphylactic reactions including fatalities, especially in patients w/ history of penicillin hypersensitivity or sensitivity to multiple allergens; rash, irritation, swelling; flu-like symptoms; blistering, skin & mucous areas (eg, oral cavity, throat, nose, genital organs & membranes) peeling off & conjunctivitis occur. Not to be used in patients w/ history of hypersensitivity. Avoid use in mononucleosis due to high risk of developing erythematous skin rash. Consider treatment discontinuation if Clostridium difficile-associated diarrhea including mild diarrhea to fatal colitis is suspected or confirmed. False +ve results for glucose urine tests w/ Clinitest (R), Benedict or Fehling soln. Severe renal impairment (ie, GFR <30 mL/min) or hemodialysis. Pregnancy (possible association w/ cleft lip w/ cleft palate). Lactation.
Adverse Reactions
Rash; diarrhea, nausea, vomiting; headache; vulvovaginitis. Erythema multiforme, SJS, TEN; Clostridium difficile diarrhea; anaphylaxis, hypersensitivity reaction.
Drug Interactions
Increased levels/effects of MTX & vit K antagonists. Increased levels/effects w/ allopurinol & probenecid. Decreased levels/effects of intravesical BCG & BCG vaccine (immunization), cholera & typhoid vaccine, lactobacillus, estriol, mycophenolate, Na picosulfate. Decreased levels/effects w/ tetracycline derivatives.
